SO 52 YEARS AGO, THERE WAS NO SUCH THING AS MEDICAID.

AT THAT TIME YOU HAD TO COME TO THE IVORY TOWER TO GET CARE.

HAD YOU TO MAKE THE BIG CHECK TO THE ACADEMIC MEDICAL CENTER.

WHAT HAPPENS IS PEOPLE WERE NOT ENGAGED IN HEALTHCARE.

THEY WERE A NUMBER.

THEY WOULDN'T MAKE THE TRIP.

THEY WOULDN'T SEEK CARE.

SO ROLL IT FORWARD 52 YEARS NOW WE HAVE MEDICAID.

WE HAVE CLINICS THAT ARE IN UNDER SERVED COMMUNITIES AND SO THE ACCESS ISSUE WAS STARTING TO GO AWAY.

SO WE GOT SCHOOL BASED CLINICS, NOW YOU START TO UNDERSTAND THE OTHER ISSUES OF HEALTHCARE.

IT'S NOT JUST ABOUT ACCESSING HEALTHCARE BUT UNDERSTANDING WHO IS THAT PERSON?

SO A DOCTOR WILL WANT TO DO A CLINICALLY BEST PRACTICE MEDICINE.

BUT HOW DO THEY APPLY IT TO THE UNIQUE SITUATION THAT EVERY PERSON IS IN.

TREATING A DIABETIC THAT'S HOMELESS IS DIFFERENT THAN TREATING A DIABETIC THAT IS IN COLLEGE.

TREATING A PREGNANT WOMAN OF CHINESE BACKGROUND IS MUCH DIFFERENT THAN TREATING SOMEONE WITH AN ENGLISH BACKGROUND.

SOME CULTURAL OWNERS MAY NOT ASSUME YOU NEED TO SEE A DOCTOR, IT'S A NATURAL CONDITION.

SO YOU ALL ONLY GO TO THE DOCTOR WHEN YOU ARE SICK.

SO AS TIME MOVES FORWARD AND WE LOOK AT THE UNITED STATES WHO SPENDS MORE AND MORE OF THEIR MONEY ON HEALTHCARE, BUT THE HEALTH OF THE POPULATION IS NOT GETTING BETTERED AND WE COMPARE OURSELVES TO OTHER COUNTRIES WHO SPEND LESS ON MEDICAL CARE AND MORE ON SOCIAL SERVICES AROUND CARE, THEY HAVE BETTER OUTCOMES.

SO THAT'S WHAT LED US TO THIS POINT.

THAT THE A FASCINATING PARADOX.

THE NOTION THAT WE SPEND MORE THAN ANYBODY.

YET WHEN THE STUDIES COME OUT IN TERMS OF WHERE WE FWAUL ON THIS LIST, IT'S ALWAYS STAGGERING TO SEE WHERE WE ARE.

SO WHEN YOU START TO THINK ABOUT AND PEOPLE WHO HAVE BEEN WORKING IN COMMUNITY HEALTH CENTRES LIKE MYSELF HAVE RECOGNIZED THIS FOR A LONG TIME WHAT WE HAVEN'T REALLY DONE IS SYSTEMATICALLY TRIED TO IDENTIFY THE FACTORS ARE THAT INTERFERE WITH THESE TREATMENT PLANS AND BRING TO BEAR RESOURCES THAT CAN HELP PEOPLE ACHIEVE THEIR GOALS.

I'LL GIVE ONE QUICK EXAMPLE.

WHAT HAVE YOU FOUND?

ALL OF YOU CAN JUMP IN ON THIS.

WHAT HAVE YOU FOUND HAVE BEEN THE MOST SIGNIFICANT BARRERS ARE TO GOOD QUALITY HEALTHCARE AND WHAT NOW ARE THE CENTERS DOING TO TRY TO GET OVER?

I'D LIKE TO START WITH TWO THINGS.

ONE IS THE WHOLE IDEA OF HOW AM I GOING TO PAY FOR THIS?

COMMUNITY HEALTH CENTER, THAT'S NOT AN ISSUE.

SO THAT'S A SOCIAL DETERMINANT HEALTH FACTOR FOR MANY PATIENTS.

WE DO THAT THROUGH FEDERAL SUPPORT AND OTHER SUPPORT.

JUST REMOVING THAT AS THE ISSUE THAT IS GOING TO AFFECT WHETHER A PATIENT CAN REALLY FOLLOW THE TREATMENT PLAN IS VERY IMPORTANT.

ANOTHER VERY IMPORTANT THING IS CULTURAL COMPETENCE.

IF A PATIENT FEELS THAT YOU ARE NOT GOING TO UNDERSTAND THEM BECAUSE THEY DON'T SPEAK THE LANGUAGE OR YOU DON'T SPEAK THEIR LANGUAGE, IF THEY COME FROM A DIFFERENT BACKGROUND THAN THE DOCTOR DOES, THEY MAY HAVE THIS PERCEPTION IF YOU WILL NOT TAKE MY CONCERN SERIOUSLY, I WON'T BE ABLE TO EXPRESS WHAT I AM TRULY GOING THROUGH, THAT NEW YORK CITY HAS MANY, MANY IMMIGRANTS AND THAT BRINGS WITH IT A LOT OF ISSUES.

WE DON'T CARE ABOUT IMMIGRANT STATUS.

WE CARE ABOUT YOU AS A PERSON.

WE'RE NOT TRYING TO, YOU KNOW, FIGURE OUT YOU KNOW WHAT YOUR CITIZENSHIP STATUS IS.

WE'RE TRYING TO MAKE SURE THAT YOU CAN BE HEALTHY SO THAT YOU CAN GET A JOB SO THAT YOU CAN PROVIDE NOR YOUR FAMILY.

AND SO I WOULD SAY THAT THOSE ARE TWO OR THREE OF THE MOST IMPORTANT ISSUES.

HOW AS A PHYSICIAN YOU CAN BE INVOLVED IN THESE SERVICES?

I WILL GIVE YOU A CLASSIC AND INCREDIBLE EXAMPLE.

WE PROVIDE I THINK WE DELIVERED 1,600 BABIES LAST YEAR AND WE HAD NOT BEEN MEANINGFULLY SCREENING WOMEN TO DETERMINE WHETHER THEY HAD ISSUES WITH HOUSING, FOR EXAMPLE, WE STARTED DOING THAT AND WE RELATIVELY QUICKLY IDENTIFIED THREE WOMEN WHO WERE PREGNANT WHO DIDN'T HAVE A PLACE TO SAY.

IT'S THE FABRIC OF THEIR LIVES.

BECAUSE THEY WERE CLEARED BY DNA EVERYDAY.

DONALD TRUMP SAYS AT THE TIME EVEN WAFER THEY WERE INDICTED THAT THEY SHOULD BE EXECUTED AND PETE HAMMILL STANDS UP TO HIM.

AT THE NATIONAL LEVEL, IT'S PRETTY MUCH ANY STORY THAT DOMINATED THE SECOND HALF OF THE 20th CENTURY, VIETNAM, WATERGATE.

INFLUENCED IN PART BY THE RAPIDLY CHANGING NEW YORK BECAUSE OF THE INFLUX OF IMMIGRANTS IN A DAY IN AND DAY OUT BASIS.

JIMMY WAS A VERY HARD-HITTING JOURNALIST.

HE HIT A DARK GLOOMY IRONIC SENSE OF THE WORLD, SHORT, PUNCHY, AND HE'D SAY THAT SORT OF CAPTURED THE ESSENCE.

PETE WAS MORE OF THE POET.

THEY DESCRIBED HIS WRITINGS, SOMETHING OUT OF A FRENCH NOVEL.

THEY TALK VERY MOVINGLY ABOUT HOW JIMMY BRESLAN WAS STANDING UP FOR BLACK PEOPLE IN THE CITY WHEN ANYBODY ELSE WOULD AND PETE ALSO WAS YOU KNOW VERY PROGRESSIVE ON A LOT OF DIFFERENT ISSUES.

JIMMY BRESLAN WAS A VERY PUGNACIOUS GUY.

IN FACT.

A LOT OF ENEMIES.

HE WROTE A COLUMN CALLED THE SMALLEST FLIGHT CAN MAKE A FEUD.

HE AND HAMMILL NEVER FEUDED.

THEY WERE NEVER RIVALS.

HOW DID THAT HAPPEN?

PUTIN WAS A VERY FORGIVING MAN.

JIMMY HAD REAL REFERENCE FOR PETE AS A SPEAKER AND A WRITER.

IN FACT, THEY WERE ULTIMATELY WONDERFUL WRITERS.

I THINK THAT JIMMY WAS PERHAPS ONE OF PETE'S TRUE FRIENDS WHO OUTLYS AND EVEN AT THE VERY WORST WHEN PETE USED JIMMY AS A SPOTTER, WHEN JIMMY USED HIM AS A SPOTTER TO WRITE A COLUMN.

PETE WAS A VERY FORGIVING FRIEND.

THIS WAS ONE OF THE FUNNIEST MOMENTS IN YOUR FILM, BRESLAN FOR SOME REASON DECIDES TO WRITE A COLUMN, WHERE HE TALKS ABOUT HOW PETE HAMMILL STEPPED OUT AND SHIRLEY McCLAIN WITH JACKIE ONASSIS.

IT'S OFTEN SAID TODAY WITH SOME CREDENCE THAT TODAY REPORTERS, DON'T REPORT BUT GIVE OPINIONS, NOT BASED ON REAL REPORTING IN CONVERSATIONS WITH REAL PEOPLE.

HE AND PETE WERE REALLY FROM THE SCHOOL OF IF YOUR MOTHER TELLS YOU, YOU LOVE YOU, CHECK IT OUT.

THEY WERE SHOE LEATHER REPORTERS.

THEY WERE HEAVY ON DETAILS.

IT WAS ABOUT THE CENTRAL PARK JOGGER.

WHEN THESE GUYS ENDED UP ACTUALLY BEING INNOCENT.

I WONDER HOW THESE TWO GUYS IF THEY HAD BEEN COVERING THE 16 PRESIDENTIAL ELECTION AND REMEMBER COVERING THIS PRESIDENCY, HOW WOULD HE LOOK LIKE?

WE WERE ASKING HIM ON THE FILM IN 2016.

AND THEY BOTH SAID THAT THEY HAVE BEEN ACTIVE, THEY WOULD BE OUT INTERVIEWING TRUMP VOTERS TRYING TO SEE WHY THEY WOULD VOTE FOR THIS THAT THIS GUY WITH HIS RECORD AND BOTH OF THEM FOR A LONG PERIOD OF TIME, WHY WOULD THEY THINK THAT THIS GUY ACTUALLY GAVE A DAM ABOUT THEM?

BUT ON THE OTHER HAND, IT'S HARD TO IMAGINE.

CORRECT ME IF I'M WRONG, JIMMY BRESLAN AND PETE HAMMILL BEING SO SURPRISE AS SO MANY REPORTERS WERE.

THAT THESE WORKERS IN THE RUST BELT TURN AWAY FROM THE ELITE.

RIGHT.

BECAUSE THEY KNEW WHAT THESE GUYS ARE SUFFERING AND SO MANY REPORTERS DON'T.

NOT ONLY THAT, BUT WE HAVE A COLUMN THAT WOULD QUOTE FROM PETE.

HE UNDERSTOOD IN THE '70s AND '80s THAT YOU NEEDED TO LISTEN TO WHAT THESE WHITE WORKING CLASS VOTERS WERE SAYING AND I THINK HE FELT THAT THE DEMOCRATIC PARTY WHICH BY AND LARGE SUPPORTED DEMOCRATS BUT HE FELT THAT THEY HAD LOST TOUCH SOME WITH THOSE VOTERS SO I THINK HE DEFINITELY WOULD HAVE BEEN WRITING ABOUT THAT.

BOTH OF THEM WOULD HAVE IN 2016.

REMEMBER JIMMY COMES FROM QUEENS.

I MEAN, HE ACTUALLY KNEW THE TRUMP FAMILY GROWING UP.

HE KNEW FRED TRUMP.

THERE IS AN UNCONFIRMED REPORT, I SHOULD STRESS, THAT FRED TRUMP, MANY YEARS AGO, TRIED TO BRIBE JIMMY INTO NOT WRITING CRITICAL COLUMNS ABOUT HIS SON.
